This form should be used by individuals who are purchasing an Institutional Membership to the Urban Affairs Association. In addition to requesting payment information, you will be asked to identify an institutional representative and provide some information about them (see below).
SECTIONS IN THIS FORM:
- Billing Information (credit card information, billing address, email address for receipt)
- Information for Membership Directory (name of institution, website URL, social media accounts)
- Your Institutional Representative - this is the person who will receive the UAA membership benefits, including access to UAA journals, a conference registration discount, and access to UAA's professional development opportunities. You will be asked to provide their:
- Name
- Email address
- Preferred way to receive UAA journal content - online OR hard copy
- If a hard copy of journal is preferred, the form will ask you to enter your representative's mailing address.
